As I noted on my Elite audio yesterday, one of the reasons that Raw received poor reviews from those who watched it may have been that the script for the show wasn't completely put to bed until just an hour or so before they went on the air.
The ending to Raw, featuring Kane's explosion was planned a week or so back, but plans as to how they would get there creatively weren't locked in until the day of the show. The angle was done with the idea of creating a "safe but dangerous looking" cliffhanger for the Cena vs. Kane storyline. The special effects involved were why the Cena segment closed the TV show, obviously.
